Your doctors and other healthcare professionals will be giving you a lot of advice on how to keep your diabetes under control. Once you are home, find a way to help you remember to eat the right foods, check your blood sugar (glucose), and take your medicines at the right time.

Now that you’ve made the decision to make healthy changes in your lifestyle, it is important to stay on track. Your healthcare professionals will help you set activity plans, diet, medication, and general healthcare goals.
Just remember to set realistic expectations that you feel you will be able to achieve. This will make it easier to reach your goals.
